misc fixes

- Sys_Warn for print(2) stuff with %r (otherwise errstr(2) blanked?)
- Sys_Error also printing %r
- in: note handler, killing input on crash
- snd: segfaults on accessing shm (no, sound still is for shit)
- vid: useless draw()
